Liability Waivers

By submitting this form, I acknowledge and accept that JAM Sports League, including its affiliated clubs, officers, and representatives, shall not be held liable for any injuries, damages, or losses—financial or otherwise—incurred by the player, team, or affiliated parties participating in activities associated with JAM Sports League of Orange County.

I understand that participation may involve playing with individuals of varying age groups and skill levels, and I voluntarily assume all risks associated with such participation. By submitting this form, I waive any right to pursue legal action against JAM Sports League or its affiliates in the event of injury or harm of any kind.

Age Mixing Acknowledgement & Liability Waiver

I understand and acknowledge that in the Adult Games hosted by Jam Sports League, some games may involve players under the age of 16 (U16) playing alongside or against adult participants, and vice versa. By registering, I accept and consent to this age mixing.

I recognize that participation in any sport carries a risk of injury and, by signing below, I voluntarily assume all such risks. I release and hold harmless Jam Sports League, its staff, volunteers, and affiliates from any liability for injuries, accidents, or incidents that may occur as a result of participation in these mixed-age games.

Media Release

By registering for Jam Sports League, I acknowledge and agree that photos and/or videos of myself or my child may be captured during league activities. These images may be taken by league staff, coaches, parents, or other participants and may be used for official purposes, including incident reports, safety documentation, and promotional content.

I grant Jam Sports League permission to use these images or videos on social media, the official website, and in other marketing or informational materials. Participation in the league constitutes consent to this media release.

I have read and agree to the Media Release. I understand that participation in Jam Sports League includes consent to the use of photos and/or videos for promotional purposes.
