By submitting this form, I acknowledge and accept that JAM Sports League, including its affiliated clubs, officers, and representatives, shall not be held liable for any injuries, damages, or losses—financial or otherwise—incurred by the player, team, or affiliated parties participating in activities associated with JAM Sports League of Orange County.

I understand that participation may involve playing with individuals of varying age groups and skill levels, and I voluntarily assume all risks associated with such participation. By submitting this form, I waive any right to pursue legal action against JAM Sports League or its affiliates in the event of injury or harm of any kind.

Age Mixing Acknowledgement & Liability Waiver

I understand and acknowledge that participants of different ages may play with or against one another. By registering, I knowingly accept and consent to this mixed-age participation.

I further acknowledge that participation in any sport involves inherent risks of injury. By signing below, I voluntarily assume all such risks and agree to release, waive, and hold harmless Jam Sports League, including its staff, volunteers, and affiliates, from any and all liability for injuries, accidents, or incidents that may arise from participation in these mixed-age activities.

Media Release

By registering for Jam Sports League, I acknowledge and agree that photos and/or videos of myself or my child may be captured during league activities. These images may be taken by league staff, coaches, parents, or other participants and may be used for official purposes, including incident reports, safety documentation, and promotional content.

I grant Jam Sports League permission to use these images or videos on social media, the official website, and in other marketing or informational materials. Participation in the league constitutes consent to this media release.
